Healthcare

Foreign nationals holding a long-stay (Type D) visa or long-term/permanent residence permit in Bulgaria are obliged to register with and pay contributions to the National Health Insurance Fund (NHIF) once their residence permit is issued, which then grants access to the state-funded public healthcare system on the same basis as Bulgarian nationals. Those on only short-term residence or without a permit are not covered and must pay for medical care out of pocket unless an international agreement applies.

NHIF obligation and access to the public system begin from the date the long-term/permanent residence permit is obtained; there is no separate long waiting period cited beyond processing of the residence permit itself, though initial months typically require private insurance while the permit is being obtained.

Private health insurance is a condition of residency in Bulgaria.

Non-EU nationals applying for a Type D long-stay visa or residence permit must show proof of private travel/medical insurance valid in Bulgaria with coverage of at least €30,000, including emergency care and hospitalization, until they are enrolled in NHIF.

US Medicare: US Medicare generally does not cover medical care received in Bulgaria; Medicare coverage is limited almost exclusively to care provided within the United States, with only very narrow exceptions (e.g., certain emergencies in bordering countries), none of which apply to Bulgaria.

Pension income tax

Bulgaria taxes foreign pension income of its residents.

Bulgaria taxes its tax residents on worldwide income, including foreign-source pensions, generally at the country's flat personal income tax rate of 10%, subject to relief under applicable double tax treaties.

There is a US income tax treaty with Bulgaria.

Under the 2007 US-Bulgaria income tax treaty, private pensions and US Social Security benefits paid to a Bulgarian resident are generally taxable only in Bulgaria (the residence state); however, the treaty's saving clause preserves the United States' right to tax its citizens and green-card holders on worldwide income regardless of residence.
